General Description

The Registered Nurse (RN) will provide nursing care and health oversight for individuals with intellectual and developmental disabilities (IDD) living in a state-funded group home. The RN will work collaboratively with the interdisciplinary team to ensure that residents receive high-quality, person-centered care, and are supported in achieving their optimal health outcomes. This position requires clinical expertise, a compassionate approach, and an understanding of the unique health needs of individuals with IDD.

Job Duties

• Conduct initial and ongoing health assessments for all residents, including monitoring vital signs, physical health, and any chronic conditions or co-occurring health issues.

• Develop, implement, and update individualized care plans for each resident, ensuring that health needs are identified and addressed.

• Oversee medication administration processes, including the review of medication orders, preparation of medications, and monitoring for any adverse effects or medication errors.

• Provide nursing interventions as needed, including wound care, catheter care, G-tube feedings, and other treatments as prescribed by physicians.

• Regularly assess the health status of residents, identifying and addressing any changes in condition, and coordinating medical care as needed.

• Work closely with residents' primary care physicians and specialists to ensure that medical needs are met, and treatments are appropriately implemented.

• Provide immediate response to medical emergencies, perform triage, and coordinate care with emergency services if needed.

• Educate residents, families, and staff on health-related topics, including chronic disease management, nutrition, hygiene, and wellness.

• Serve as a health advocate for residents, ensuring their healthcare rights and needs are met. Act as a liaison between residents and healthcare providers.

• Train and supervise direct care staff in health-related areas, including medication administration, infection control, and recognizing signs of illness or distress.

• Maintain accurate, up-to-date medical records for each resident, including care plans, progress notes, and incident reports.

• Ensure that all nursing activities comply with Maryland state regulations, Medicaid waiver programs, and other applicable healthcare laws and guidelines.

• Monitor and document any health-related incidents, including falls, injuries, or medication errors. Implement corrective actions as necessary to prevent recurrence.

• Participate in regular quality assurance activities, including internal audits and reviews, to ensure the quality of care provided to residents.

• Other duties as assigned.

Qualifications

Knowledge & Abilities

• Strong clinical assessment skills and the ability to develop and implement care plans.

• Excellent communication and interpersonal skills.

• Proficiency in managing and administering medications.

• Ability to work independently and as part of an interdisciplinary team.

• Proficiency in electronic medical records (EMR) systems and Microsoft Office Suite.

Education, Licenses, & Training

• Associate or Bachelor’s degree in nursing from an accredited program.

• Current, active RN license in the state of Maryland.

• Certification in developmental disabilities nursing (C-DDN) or other relevant certifications preferred.

• 2+ years of nursing experience, preferably in a group home, long-term care, or IDD setting.

• Experience with individuals with intellectual and developmental disabilities is highly preferred.



Additional Information

  • Shift

    • Monday – Friday 9am – 5:00pm and flexibility to accommodate individual needs and schedule variations.
