The RN II Advanced Primary Care position focuses on delivering virtual care to patients who require ongoing management of chronic diseases. Collaborating with a diverse team that includes physicians and advanced practitioners, the RN will foster patient relationships, track vital sign trends, conduct interim patient visits, assist in managing patient panels, and offer personalized care aimed at enhancing patient outcomes. This role reports to the designated Advanced Primary Care Lead APP.

Location: Hybrid work model after training; candidates must reside within one hour of the Chesterfield Virtual Care Office.

Key Responsibilities:

As an RN II in Advanced Primary Care, you will collaborate closely with a multidisciplinary team—including physicians and advanced practitioners—to provide continuous support for patients with chronic conditions. Your duties will encompass:

  • Establishing strong relationships with patients and monitoring their vital sign trends.
  • Conducting virtual consultations between regular office visits.
  • Assisting with chronic disease management, follow-up after discharge, and addressing acute care needs.
  • Promoting guideline-directed medical therapy (GDMT) and facilitating advance care planning for Medicare and MA patients.
  • Working alongside primary care clinicians in outpatient settings to ensure coordinated care.
  • Performing virtual assessments, providing patient education, triaging, and escalating care needs as necessary.
  • Evaluating medically complex patients and managing acute exacerbations of chronic conditions.
  • Coordinating care with a multidisciplinary team, including specialists, social workers, physical therapists, and palliative care providers.
  • Documenting all care provided while ensuring patient confidentiality.
  • Practicing within the RN scope under the guidance of an advanced practitioner.

Qualifications:

  • Current RN license in the relevant state(s).
  • Preferred experience in chronic disease management and virtual care.
  • Excellent communication and teamwork abilities.
