Key Responsibilities
- Conduct comprehensive physical therapy assessments to evaluate patients' mobility, strength, range of motion, and functional abilities
- Develop and implement individualized treatment plans tailored to each patient's specific rehabilitation goals and medical conditions
- Deliver evidence-based physical therapy interventions including therapeutic exercises, manual therapy, gait training, and mobility assistance
- Provide patient and family education on home exercise programs, proper body mechanics, fall prevention, and adaptive equipment use
- Collaborate effectively with physicians, nurses, occupational therapists, and other healthcare team members to ensure coordinated care
- Maintain detailed, accurate documentation of patient progress, treatment plans, and outcomes in compliance with regulatory standards
- Monitor patient progress and adjust treatment protocols as needed to optimize rehabilitation outcomes
- Ensure patient safety and infection control practices during all home visits
Required Qualifications
- Current, unrestricted Physical Therapist (PT) license in the State of Texas
- Doctor of Physical Therapy (DPT) degree from an accredited program
- Comprehensive knowledge of physical therapy principles, techniques, and best practices
- Excellent verbal and written communication skills with patients, families, and healthcare professionals
- Strong clinical reasoning and critical thinking abilities
- Ability to work independently with minimal supervision while managing a flexible schedule
- Compassionate, patient-centered approach with excellent interpersonal skills
- Valid driver's license and reliable transportation for home visits throughout the service area
- CPR certification (or willingness to obtain upon hire)
Preferred Qualifications
- Previous home health physical therapy experience strongly preferred
- Experience working with geriatric populations and chronic conditions
- Familiarity with Medicare/Medicaid documentation requirements
- Bilingual skills (English/Spanish) are a plus
