About the Position

  • Community Health Worker - Substance Use Disorder
  • Part-time .8 FTE (64 hours per pay period) to last approximately 11 months. The role's functions and will be assessed prior to the eleven-month mark to determine continuation of FTE.
  • On-site at Grand Itasca Clinic & Hospital in Grand Rapids, MN.

Job Summary:

The Community Health Worker (CHW) is responsible for helping community members, patients and their family's access and navigate community services and resources and adopt healthy behaviors, specifically with individuals struggling with substance use disorder (SUD). The role also supports patients and families experiencing neonatal abstinence syndrome (NAS). The CHW supports providers and team members through an integrated approach to care management and community outreach. The role's actions will promote, maintain, and improve individual and community health. The CHW provides social support and informal counseling, individual and group health education and advocacy for individuals and community health needs. Providing home visits and health screenings may be required. This position is a part of a Health Resource Services Administration (HRSA) Rural Communities Opioid Response Program (RCORP) grant received by Grand Itasca Clinic & Hospital, where funding will be used to strengthen and expand SUD/opioid use disorder (OUD) prevention, treatment, and recovery service delivery in our community.


The grant has eleven months remaining on the monetary payment term. The sustainability of this role will be assessed prior to the eleven-month mark to determine continuation.


RCORP Description:

Over the funding period, Grand Itasca's RCORP project will:

  1. Reduce structural and systems-level barriers within the rural target service area to
    1. Increase access to behavioral health care, specifically for those experiencing SUD/OUD, and more explicitly, for rural pregnant and postpartum persons and their families; and
    2. Address community risk factors and social determinants of health.
  2. Strengthen the quality and sustainability of behavioral health care services for those experiencing SUD/OUD, focusing in on rural pregnant and postpartum persons and their families in the service area by implementing coordinated, evidence-based, family-centered SUD/OUD and other services.


Here's what you'll do when you join us:


  1. Provide Direct Services for individuals experiencing SUD: Promote wellness by providing culturally appropriate health information to providers, patients and/or community members. Provide individual social and health care support. Refer and link to preventive services through health screenings and healthcare information. Conduct health related screenings as assigned
  2. Navigate the health and human services system: Increase access to primary care through culturally competent outreach and enrollment strategies. Make referrals and coordinate services. Teach people the knowledge and skills needed to obtain care. Facilitate continuity of care by providing follow-up. Link patients to and inform them of available community resources
  3. Advocate for individual and community needs related to SUD: Articulate and advocate needs of community and individuals to others. Be a spokesperson for patients when they are unable to speak for themselves. Involve participants in self and community advocacy. Map communities to help locate and support needed services
  4. Bridge the gap between communities and the health and social service systems: Enhance care quality by aiding communication between health system, patient, and community members to clarify cultural practices. Educate patients/community members about how to use the health care and social service systems. Educate the health and social service systems about patient/community needs and perspectives. Establish better communication processes
  5. Build Individual and Community Capacity around SUD: Build individual capacity to achieve wellness. Build community capacity by addressing social determinants of health. Identify individual and community needs. Seek professional development (continuing education)


If you have these qualifications, we'd love to chat:

  1. College degree in health, human services or related field.
  2. Three years' experience working on community health projects, programs or initiatives.
  3. Experience, demonstrated competency, and sensitivity in working with diverse populations.
  4. Community based leadership experience.
  5. Community Health Worker Certificate will be required within 18 months of hire.
  6. Demonstrates respectful and effective communication skills with patients, families, providers, and staff.
  7. Demonstrated ability to effectively problem solve in difficult situations.
  8. Demonstrated ability to follow and adhere to Grand Itasca Clinic and Hospital policies, vision, mission and values.
  9. Maintains reliable and consistent attendance.
