KeystoneCare is seeking a dedicated and passionate Intake Coordinator to join our team! As the initial point of contact for patients and referral sources, you will play a crucial role in our organization by guiding individuals through the healthcare entry process.

In this role, you will be responsible for educating patients about our services, managing referrals, streamlining case assignments to the clinical team, and ensuring the continuity of care. Your contributions will have a direct impact on the lives of our patients and their families.

Primary Responsibilities:

  • Receive and process referrals through phone calls and referral networks.
  • Inform patients and referral sources about the agency's offerings effectively.
  • Evaluate the agency's capacity to accept referrals based on location, service needs, and insurance.
  • Input detailed information into our Electronic Medical Record system.
  • Allocate patient cases thoughtfully to clinical teams based on patient requirements and team availability.
  • Ensure that patient care initiation meets agency and regulatory timelines.
  • Maintain consistent communication with patients, caregivers, and referral sources regarding care initiation and changes.
  • Adjust staff schedules to accommodate planned and unplanned time off.
  • Deliver a 5-star experience for all consumers seeking care.
  • Adhere to directions from leadership and maintain strict confidentiality of patient information.
  • Perform additional tasks as assigned.

Requirements

  • Minimum 3 years of relevant experience in a healthcare setting, preferably in home health or hospice.
  • Ability to multitask in a dynamic, fast-paced setting while maintaining effective communication.
  • Strong customer service skills with a proven capacity to respond to the needs of patients and healthcare professionals.
  • Exceptional data entry and scheduling skills.
  • Self-motivated and capable of working independently as well as collaboratively within a healthcare team.
  • Technologically proficient, with experience using electronic documentation systems.
  • Proficient with Microsoft Office Suite (Word, Excel, Outlook).
  • Must provide proof of COVID-19 and Influenza (Flu) vaccinations, or an approved Medical or Religious exemption.
