- Manage a caseload and complete member assessments in home, community, or telephonic settings.
- Provide clinical support and collaborate with the Interdisciplinary Care Team (ICT) for members with medical complexity.
- Develop, implement, and coordinate individualized care plans to ensure appropriate services and address barriers to care.
- Conduct medical attestations, clinical reviews, and ensure services are provided in the least restrictive setting.
- Support members and caregivers in accessing social, housing, educational, and other community resources.
- Serve as the primary point of contact for the ICT and coordinate with internal and external stakeholders to meet member needs.
What You’ll Bring:
- Michigan LBSW or LLBSW (MSW preferred)
- 2+ years in behavioral health or clinical care management
- Excellent communication and coordination skills
- Valid driver’s license, reliable car, and home office with internet
