Job Description
Pay:
- PTAs are paid per visit using a point-based system. Each visit type is assigned a point value, and your pay is based on your Point Rate x Number of Points you complete in a week.
- No limit on how many patients you see in a week!
Qualifications:
Required:
- Graduate from PTA program
- PTA License
- CPR Certification
- Active and Valid Driver’s License and Auto Insurance
- At least 1 year of Home Health or Skilled Nursing Facility (SNF) experience
Preferred:
- Homecare Homebase EMR experience
- Multiple years of Home Health or Skilled Nursing Facility (SNF) experience
Job Summary:
- Provides physical therapy services under supervision of a licensed physical therapist and in line with physician’s plan of care.
- Delivers treatment to relieve pain, restore function, and maximize patient performance.
- Guides patients through active and passive exercises, gait training, functional training, transfers, and muscle reeducation.
- Administers physical agents such as heat, cold, water, light, ultrasound, massage, and electrical stimulation.
- Monitors patient response to therapy and reports progress or changes to the physical therapist and care team.
- Educates patients on safe use of mobility aids, prosthetics, orthotics, and adaptive equipment.
- Maintains accurate and timely clinical documentation for inclusion in patient records.
- Participates in interdisciplinary care team meetings and continuing education programs.
- Performs additional duties as assigned to support patient care and team operations.
- Adheres to attendance and reliability standards to ensure consistent, high-quality care delivery.
