Part-Time Nurse Practitioner - Home Care & Skilled Nursing Facilities
Fall River, MA | Part-Time, 32 hours/week | Competitive Compensation
Key Responsibilities:
•Conduct home visits: assess conditions, reconcile medications, educate patients and caregivers, order tests as appropriate
•Collaborate with visiting nurses, primary care physicians, and specialists to coordinate care
•Round at skilled nursing facilities, working alongside NPs and facility medical directors to develop management plans and direct patient care
•Support efficient use of home nursing and therapy resources and help prevent hospital readmissions
Qualifications:
•Licensed Nurse Practitioner in Massachusetts (Rhode Island license preferred)
•Valid DEA certificate and Massachusetts controlled substances registration
•Strong clinical judgment, organization, and problem-solving skills
•Ability to work independently and collaborate with nurses, therapists, pharmacists, administrators, case managers, and physicians
•Fluency in Portuguese is a plus, but not required
•Several years of NP experience preferred; recent graduates considered if confident in independent practice
