Location: Hudson, Florida

Compensation: Details provided to qualified applicants

Schedule: 8-hour day shifts, five days per week; every other weekend and holiday rotation

Highlights:

• Work in a 309-bed Level II Trauma Center with a robust acute care environment
• Manageable patient ratios: 1:25 on medical-surgical floors, 1:20 in progressive care units
• Utilize multiple systems including Meditech for documentation, Midas for case management, and CarePort for referrals
• Full-time position with consistent day shift hours
• Collaborative, multidisciplinary team environment with opportunities to lead care coordination discussions
• Access to modern technology including iPad, computer, and mobile communication tools

Requirements:

• Active Florida RN license or ability to obtain prior to start date
• Hospital and/or acute case management experience required
• Experience with Midas, InterQual, and Meditech preferred
• Proficiency in discharge planning, assessments, and patient evaluations
• Strong critical thinking, problem-solving, and facilitation skills
