Position Overview

As a Home Health Physical Therapist, you will:

• Assess patient mobility and develop personalized treatment plans.
• Collaborate with a multidisciplinary team to ensure comprehensive care.
• Deliver therapy interventions as directed by the physician.

Essential Job Functions

In this role, you will:

• Conduct initial and ongoing assessments to evaluate mobility, coordination, and strength.
• Administer, evaluate, and interpret tests to set functional goals.
• Provide written evaluations and plans of care that define therapy needs and expected outcomes.
• Document client performance and modify treatment plans as necessary.
• Plan for patient discharge, educating them and their caregivers on home exercise plans.
• Coordinate services with healthcare teams, including physicians and nurses.
• Supervise support staff and ensure compliance with policies and procedures.

Requirements

To thrive in this role, you should have:

• A degree in Physical Therapy from an accredited university/program.
• A current, unrestricted state license as a Physical Therapist.
• Proficiency in clinical assessments and documentation.

Preferred Qualifications

• Strong organizational and communication skills.
• Attention to detail and effective time management.
• Experience in Home Health Therapy and OASIS documentation.
