PRN Registered Nurse (RN) – Home Health Visit Nurse / Wound Care
Job Description:
Atlantic Care Services is seeking an experienced PRN Registered Nurse (RN) to provide skilled, in-home nursing visits throughout the Phoenix metro area.
This position is ideal for an independent, reliable RN with strong home health and wound-care experience who can respond quickly to new patient needs.
Pay: $100 per completed visit, plus mileage reimbursement.
Responsibilities:
- Perform skilled nursing visits in patients’ homes.
- Complete wound assessments, wound care, dressing changes, and wound-vac care.
- Conduct nursing assessments, including medication reconciliation and patient-condition monitoring.
- Provide medication administration as ordered.
- Teach and train patients, families, and caregivers on wound care, medications, disease management, equipment, and care plans.
- Communicate changes in condition promptly to the physician, clinical supervisor, and care team.
- Complete accurate, timely visit documentation.
- Follow physician orders, agency policies, infection-control standards, and applicable nursing regulations.
- Accurately maintain EMR documentation
- Coordinate as needed with physician offices and case managers.
Requirements:
- Active Arizona RN license in good standing.
- Prior home health visit experience required.
- Strong wound-care experience required; wound-vac experience strongly preferred.
- Comfortable working independently in patient homes.
- Able to see a patient within 24 hours of notification when assigned a visit.
- Reliable transportation, valid driver’s license, and current auto insurance.
- Current CPR/BLS certification.
- Strong communication, clinical judgment, and documentation skills.
Schedule:
PRN / as-needed visits. Flexible scheduling, with the ability to accept and complete assigned visits promptly.
