Responsibilities

- Evaluate patient functional status and therapy needs
- Develop and implement PT plan of care with physician collaboration
- Provide skilled physical therapy services in the home
- Monitor patient progress and report status changes
- Restore function, manage pain, and maximize mobility
- Educate patients, families, and care team on treatment goals and progress
- Complete OASIS visits and add-on evaluations

Required

- Current unrestricted Physical Therapist license
- Current CPR certification
- Valid driver's license, auto insurance, and reliable transportation
- Ability to provide care in home health settings
