This position requires working in or near the location of the posting. This role does require traveling to the homes and in the community of the clients we serve.

We are actively seeking a dedicated Care Manager to serve the Broomfield, Boulder, Adams, and Wheat Ridge areas.

Behavioral Health
- Conducts brief behavioral health assessments and screening to determine eligibility, treatment needs, and treatment options.
- Assists in providing support for clients daily activities of daily living and parenting supports in a manner that promotes positive relationships, structure, and routines with families.
- Assists clients in establishing goals and helping the clients in achieving individualized goals created in their treatment plan.
- Crisis intervention.
- Facilitates substance use groups and psychoeducational supports.
Case Management
- Directs the client in arranging necessary community support services, such as mental health services, public assistance, vocational training, and similar.
- Documents all direct and indirect contacts with the youth, their family, and community support services.
- Service plan development specifying goals and actions to address member needs, ensuring participation, and identifying a course of action; includes transition plan development.
- Submits Progress Summaries to the referring agencies with ongoing recommendations and makes weekly contact with these agencies.
- Performs other duties as assigned.

Knowledge, skills, and abilities:
- Strong written and oral communication skills via phone and face-to-face.
- Understand HIPAA, protected health information, and confidentiality.
- Proven ability to work with a high level of independence, maintain confidentiality, prioritize assignments, and manage time effectively.
- Be committed to building trust and engagement with individuals and families from diverse backgrounds.
- Strong conflict resolution skills.
- Practice cultural awareness and a commitment to diversity and equity.
- Demonstrates rational decision-making skills.
- Knowledge of parenting skills, childhood developmental stages, and similar.
Education:
- A confirmed bachelors degree in a relevant field in Counseling, Social Work, or related field is required.
Experience:
- One year of relevant experience in Behavioral Health and/or Case Management is preferred.
- A bi-lingual (Spanish speaking) applicant is preferred depending upon position location.
Additional Requirements:
- A valid drivers license, reliable vehicle, and proof of automobile insurance are required.
- Must live in the communities we serve.

Salary: $26.44-31.25 (Equivalent to $55,000-$65,000/year)

Schedule:
- Choose your own hours
- Required to provide 2-3 evenings per week between 4-7 pm or 3-4 hours on a weekend (Sat or Sun.) to ensure our capacity to see children/adolescents.
