Solstice Hospice & Palliative Care is a locally owned home health & hospice dedicated to providing compassionate, responsive, and clinically excellent home care and end-of-life care. We pride ourselves on timely admissions, strong interdisciplinary collaboration, and hands-on clinical leadership. We are seeking aFull-Time RN Intake / Clinical Coordinator to support our growing census and ensure seamless transitions from hospital/SNF to home health & hospice care.
This role is ideal for an experienced hospice or home health nurse who is highly organized, clinically strong, and thrives in a fast-paced, collaborative environment.
Position Overview
The RN Intake / Clinical Coordinator is responsible for managing the hospice intake and admissions process while providing clinical coordination support to the interdisciplinary team. This role serves as a key clinical and operational bridge between referral sources, patients, families, and internal care teams to ensure timely, appropriate, and high-quality hospice admissions.
Responsibilities
- Manage hospice intake referrals, including initial clinical review and eligibility screening
- Coordinate and facilitate timely hospice admissions in collaboration with physicians and clinical staff
- Conduct or support hospice admissions as needed
- Communicate effectively with referral sources, hospitals, facilities, patients, and families
- Ensure accurate, timely clinical documentation and compliance with hospice regulations
- Support care coordination across nursing, social work, chaplaincy, and aide services
- Participate in interdisciplinary team meetings and clinical planning
- Provide clinical guidance and support to field staff as assigned
- Assist with quality assurance, census management, and operational workflows
Qualifications
- Current Utah RN license (required)
- Hospice or home health experience strongly preferred
- Strong clinical assessment and decision-making skills
- Excellent organizational, communication, and documentation abilities
- Ability to manage multiple priorities in a time-sensitive environment
- Patient-centered, calm, and professional approach
- Valid drivers license and reliable transportation
