The nursing process is utilized to deliver patient care under the supervision of individuals licensed by a State regulatory board to prescribe medications and treatments, or under the direction of a Registered Nurse (RN). The LPN maintains professional accountability for the care provided to assigned patients, coordinating efforts with various departments. 12-Hour Shifts Position Summary
The Licensed Practical Nurse (LPN) is responsible for delivering high-quality patient care under the supervision of licensed professionals authorized to prescribe medications and treatments or under the direction of a Registered Nurse (RN). This role ensures safe, compassionate, and effective care by employing the nursing process, maintaining professional accountability, and collaborating with multidisciplinary teams. The LPN is committed to upholding patient rights and confidentiality while performing duties in accordance with our mission, values, and service standards.
Key Responsibilities
- Assist the RN with the admission and discharge processes.
- Conduct patient assessments and promptly communicate any changes.
- Facilitate patient handoffs using safety protocols (e.g., SBAR, bedside report).
- Work with the RN to assess patient progress and document interventions.
- Support patient and family education and document learning needs.
- Delegate tasks to Unlicensed Assistive Personnel (UAPs) and ensure care is delivered within their competencies.
- Complete necessary competencies, certifications, and engage in process improvement initiatives.
- Act as a preceptor for LPN students or new hires as needed.
- Take on additional responsibilities as assigned to enhance patient care and departmental objectives.
Qualifications
- Graduate from an accredited practical nursing program.
- Current LPN license in the state of practice.
- Basic Life Support (BLS) certification within 30 days of hire.
- LPN-IV certification upon hire or within the required timeframe.
- Additional certifications may be necessary based on the unit (ACLS, PALS, PEARS, etc.).
Physical Requirements
- Capability to push, pull, and lift up to 50 lbs regularly.
- Prolonged periods of standing and walking during shifts.
- Frequent bending, kneeling, reaching, twisting, and squatting.
