Options for Community Living, Inc. is looking for a Medical Case Manager to join our team! An ideal candidate should meet the following requirements:
- Bachelor's degree in health, human services or nursing preferred and 1 year of qualifying experience* OR Associate's degree (OR 60 credit hours of college study) in health, human services, or education, OR LPN, OR CASAC, and 2 years of qualifying experience.*
- Valid driver's license, safe driving record, valid auto insurance and access to a vehicle is required.
*Qualifying Experience: verifiable full or part-time case management or case work with persons with HIV or AIDS, and/or persons with history of mental illness, homelessness, or chemical dependence, or other populations of persons in need.

Salary Range: $44,850/yr. ($23.00/hr.) - $58,500/yr. ($30.00/hr.)
The above salary range represents Options for Community Living's good faith and reasonable estimate of potential compensation that may be offered to a successful applicant for this position at the time of this job posting and may be modified in the future. When determining a salary offer, several factors may be considered as applicable (e.g., years of relevant experience, education level, language skillset, credentials, professional licensure, budget, and internal equity).
Schedule Options: (37.5 hours/week)
- Monday - Friday: 8:00 AM - 4:00 PM (30 minute break)
- Monday - Friday: 8:00 AM - 4:30 PM (1 hour break)
- Monday - Friday: 8:30 AM - 5:00 PM (1 hour break)
- Monday - Friday: 9:00 AM - 5:00 PM (30 minute break)
Location: In person based out of our Ronkonkoma & Hempstead offices, with field visits required in Nassau & Suffolk Counties.
Pay Type: Non-exempt
Responsibilities:
The Medical Case Manager (MCM) is responsible for providing medical case management and treatment adherence for residents. The MCM advocates for residents to obtain medical and the full range of needed support services and ensures coordination of such services. The MCM promotes linkage development and monitors the effectiveness of linkages with other service providers. The MCM ensures community follow-up to engage the client in care, promotes client compliance with medical appointments, medication and treatment adherence and encourages client self-sufficiency and empowerment.
- Ensure all contacts and documentation is completed in a timely matter and up to date.
- Obtain referrals for medical care and communicate resident's needs with the Supportive Housing Team.
- Perform home visits to residents and attend medical appointments.
- Educate client and the Supportive Housing Team on treatment adherence, health and human service resources, assist in obtaining services, and follow-up on service delivery.
- Interact with service providers in a professional manner while advocating for residents.
- Appropriately intervene in situations requiring immediate attention to ensure safety of residents.
- Maintains knowledge of resident's medical insurance. Ensure proper billing information is provided at each medical visit.
- Assist with selecting an MLTC and assist with the enrollment process.
- Coordinate referrals to Home Health Care and assisted living facilities.
- Must use own vehicle to travel to meet residents.
Salary Range$44,850—$58,500 USD
