Job Overview
Comfort Creek Nursing & Rehab Center is seeking a compassionate and motivated Physical Therapist (PT) to join our growing in-house rehabilitation team. We are hiring for Full-Time, Part-Time, and PRN (Per Diem) positions with flexible scheduling options designed to support work-life balance.
As a Physical Therapist, you will evaluate residents, develop individualized treatment plans, and provide evidence-based therapy services that help restore mobility, reduce pain, improve function, and maximize independence. You will work collaboratively with an experienced interdisciplinary team dedicated to delivering exceptional rehabilitation services in our skilled nursing and subacute rehabilitation setting.
Essential Duties and Responsibilities
- Evaluate residents and develop individualized physical therapy treatment plans based on physician orders, clinical findings, and resident goals.
- Provide skilled physical therapy interventions to improve strength, balance, mobility, endurance, gait, coordination, transfers, and overall functional independence.
- Assess and treat residents with orthopedic, neurological, cardiopulmonary, and medically complex conditions.
- Monitor resident progress and modify treatment plans to maximize clinical outcomes.
- Complete accurate and timely documentation, including evaluations, daily treatment notes, progress reports, recertifications, and discharge summaries in accordance with Medicare, Medicaid, state, federal, and facility requirements.
- Collaborate with nursing, occupational therapy, speech therapy, physicians, social services, dietary staff, and other interdisciplinary team members.
- Educate residents, families, and caregivers on therapeutic exercises, mobility strategies, fall prevention, home safety, and assistive device use.
- Participate in care conferences, discharge planning, and interdisciplinary team meetings.
- Recommend, fit, and train residents in the use of mobility aids, adaptive equipment, and assistive devices.
- Maintain compliance with all regulatory, professional, and facility standards.
- Promote a positive, resident-centered rehabilitation environment focused on quality outcomes and clinical excellence.
QualificationsRequired
- Graduate of an accredited Physical Therapy program.
- Current Georgia Physical Therapist (PT) license in good standing.
- Current CPR certification or willingness to obtain upon hire.
- Strong clinical evaluation, treatment, and communication skills.
- Ability to work effectively within a collaborative interdisciplinary rehabilitation team.
Preferred
- Experience in skilled nursing, subacute rehabilitation, long-term care, inpatient rehabilitation, or post-acute care.
- Knowledge of Medicare documentation and reimbursement guidelines.
- Experience treating geriatric and medically complex populations.
- Familiarity with evidence-based rehabilitation practices, fall prevention, and functional mobility training.
