Residential Case Manager
The Residential Case Manager plays a key role in supporting clients during withdrawal management and early recovery in our Residential Treatment Center. This position provides client-centered, recovery-oriented services with a focus on care coordination, discharge planning, daily engagement, and removing barriers to continued treatment.
Working as an integral part of the clinical team, the Case Manager ensures smooth transitions across levels of care and connects clients with appropriate community resources to support their long-term recovery.
Key Responsibilities
- Provide client-centered case management and support services.
- Manage a caseload and help clients overcome barriers such as housing, employment, transportation, and medical or legal needs.
- Work closely with counselors, medical staff, and supervisors to support treatment plans.
- Encourage recovery goals, life skills, and participation in daily programming.
- Connect clients to community resources, follow-up services, and appropriate levels of care.
- Engage with clients daily, offering emotional support and crisis intervention when needed.
- Advocate for client needs during detox and early stabilization.
- Lead discharge planning and ensure smooth transitions to ongoing treatment.
- Communicate with external providers to confirm appointments and placements.
- Maintain accurate and timely documentation in line with agency and regulatory standards.
- Uphold all confidentiality requirements, including HIPAA and 42 CFR Part 2.Collaborate with internal teams and community partners.
- Stay informed on best practices in recovery and community services.
- Assist with program development and general program operations as needed.
Qualifications
- High School Diploma or Equivalent, Associates preferred
- Active CDCA (Chemical Dependency Counselor Assistant)
- Ability to pass BCI, Corporate Compliance checks, and employment drug screening
- Knowledge of recovery principles, behavioral health systems, and community resources
- Strong rapport-building skills with diverse populations
- Effective problem-solving, organizational, and time-management skills
- Ability to verbally de-escalate crises and maintain professional boundaries
- Experience collaborating with multidisciplinary teams
- Strict adherence to confidentiality standards
- Preferred: 2+ years of experience in case management, substance use treatment, or social services
