Position: Physical Therapist (PT) - Home Health
Location: Phoenix, AZ - West Valley
Compensation: $140,000 annual base + overage bonus + $2,000 sign-on bonus!
Schedule: Days | 36-40 hours/week | Flexible scheduling
Employment type: Direct Hire/Permanent role
Overview: The Home Health Physical Therapist will provide skilled, in-home physical therapy services to patients throughout the West Valley territory (area west of I-17), delivering individualized treatment plans that support patient recovery, mobility, and independence within the home setting.
Qualifications

Current, unrestricted Arizona PT license (or eligibility to obtain)

Home Health experience required

Valid driver's license, reliable transportation, and willingness to travel within the West Valley territory

Strong clinical, organizational, and communication skills

Ability to work independently in a home-based care setting
