Job Summary:


  • Unit Notes
  • - Unit: PT, OT, Speech - Marian & Arroyo Grande (home based care)
  • - Certs: Speech - do they prefer/require CCC-SLP and/or CCC-A credential BLS required
  • - EMR: Cerner
  • - Rehab Aides/OTA/PTA: yes - usually will be perm staff though
  • - Weekend Requirement: every 3rd weekend
  • - Holidays: minor holidays, major only if needed
  • - Scrub Color/Uniform: business attire with white lab coat, no jeans
  • - Outpatient and Inpatient: N/A
  • - Shifts: flexible, majority of work hours are M-F 8am-5pm
  • - What areas are they required to cover: they will be assigned to a geographical region
  • - Floor specific orientation: 1.5-2 weeks
  • - Common diagnosis / Types of patients: PT/OT - ortho, strokes, ortho, joints, spinal injuries speech - anything
  • - Additional Notes: Candidates must have experience with Oasis (gov't mandated form). Must also have one year experience working PT/OT/Sp, home setting experience is preferred but not required.




We are seeking a compassionate and licensed Physical Therapist (PT) to join our team. The ideal candidate will assess patients, develop individualized treatment plans, and implement physical therapy interventions to help patients improve mobility, manage pain, and restore physical function. This role involves working closely with physicians, patients, and families to ensure high-quality care and optimal recovery outcomes.

Key Responsibilities:


  • Perform thorough evaluations of patients physical abilities and limitations.
  • Develop and implement customized treatment plans based on diagnoses and patient goals.
  • Use therapeutic exercises, hands-on therapy, and equipment to aid patient recovery.
  • Educate patients and their families on home exercises and prevention strategies.
  • Monitor patient progress, adjust treatments as needed, and document all findings in medical records.
  • Collaborate with physicians and other healthcare team members for coordinated care.
  • Maintain compliance with federal, state, and professional regulatory standards.
