Pride Health is actively hiring a Physical Therapist (PT) to join our client's Home Health Team.
\n
\n
Location: Lewisburg, WV 24901
Pay Rate: $61.00/hour on W-2
Weekly Hour Guarantee: 32 hours
Productivity: 30 points
Patient Care Setting: Home Health
Coverage Area: Greenbrier, Pocahontas, and Monroe Counties
Schedule: Monday–Friday, 8:00 AM–4:30 PM
On-Call: No on-call hours
Weekend: Rotating weekend coverage among Physical Therapists
Mileage: No mileage reimbursement between visits
Dress Code: Navy Blue

Position Summary
\n
We are seeking a Physical Therapist (PT) to provide skilled physical therapy services to patients in their homes across Greenbrier, Pocahontas, and Monroe counties. The PT will evaluate patients, develop individualized treatment plans, provide therapeutic interventions, and monitor progress to support safe and effective recovery.

Key Responsibilities
\n

- Conduct comprehensive physical therapy evaluations and assess patients' functional abilities.
- Develop individualized treatment plans based on patient needs and established goals.
- Provide skilled PT interventions and therapeutic exercises in the home setting.
- Monitor patient progress and modify treatment plans as clinically appropriate.
- Educate patients and caregivers on exercises, mobility, safety, and home programs.
- Document patient assessments, treatments, progress, and outcomes accurately and timely.
- Coordinate care with physicians, nurses, therapists, and other members of the interdisciplinary team.
- Manage a caseload while meeting the expected 30-point productivity requirement.
- Participate in the rotating weekend coverage schedule.
- Provide compassionate, patient-centered care while maintaining professional standards.

Required Qualifications

- Current Physical Therapist (PT) license required.
- Ability to provide independent care in a home health environment.
- Strong clinical assessment, communication, and organizational skills.
- Ability to travel throughout the assigned coverage area.
