Compensation: Up to $1,700 Weekly

Job Responsibilities
\n\n
- \n\t

Provide direct patient care to residents in a swing bed and skilled nursing setting
\n\t\n\t
- \n\t
Monitor patient conditions, vital signs, and overall health status
\n\t\n\t
- \n\t
Administer medications and treatments as prescribed by physicians
\n\t\n\t
- \n\t
Assist with patient assessments and document care accurately in medical records
\n\t\n\t
- \n\t
Collaborate with Registered Nurses, physicians, and interdisciplinary care teams
\n\t\n\t
- \n\t
Support patients with daily living activities while maintaining dignity and comfort
\n\t\n\t
- \n\t
Observe and report any changes in patient condition to supervising staff
\n\t\n\t
- \n\t
Ensure compliance with facility policies and state nursing regulations
\n\t\n

Education & Certification Requirements
\n\n
- \n\t
- \n\t
Active Illinois LPN License (or compact eligibility if applicable)
\n\t\n\t
- \n\t
Graduate of an accredited Practical Nursing program
\n\t\n\t
- \n\t
BLS Certification required
\n\t\n\t
- \n\t
Previous skilled nursing, swing bed, or long-term care experience preferred
\n\t\n\t

Strong clinical assessment and patient care skills
\n\t\n\t

Excellent communication and teamwork abilities
