Hospice Registered Nurse Case Manager
Location: New Harmony, IN
Job Type: Full-Time
Schedule: In-person | On-call participation required
Position Overview
We are seeking a compassionate and dedicated Hospice Registered Nurse Case Manager to provide high-quality, patient-centered hospice care for patients and families throughout Southern Indiana and Southern Illinois. This role focuses on delivering skilled nursing care, coordinating interdisciplinary services, and supporting patients through every stage of end-of-life care with dignity, compassion, and professionalism.
Key Responsibilities
• Provide direct hospice nursing care and case management services
• Perform patient assessments and develop individualized care plans
• Coordinate care with physicians and interdisciplinary team members
• Educate patients and families on hospice care, medications, and treatment plans
• Maintain accurate and timely clinical documentation
• Monitor patient conditions and communicate changes appropriately
• Participate in quality improvement initiatives and on-call coverage
• Support patients and families with emotional and end-of-life care needs
Coverage Area
This position supports patients across multiple counties in Indiana and Illinois, including:
• Indiana Counties: Vanderburgh, Posey, and Gibson
• Illinois Counties: White, Wabash, Edwards, Lawrence, and Saline
All coverage areas are located within proximity to one another, and travel reimbursement is provided for eligible work-related travel.
Qualifications Required
• Graduate of an accredited nursing program
• Active RN licensure in Indiana and Illinois required
• Minimum one (1) year of experience in hospice, home health, oncology, or long-term care
• CPR certification required
• Valid driver's license and current automobile insurance
• Strong communication, patient assessment, and care coordination skills
• Experience or certification related to dementia care preferred
