Job Summary

The Preventative Health & Care Coordinator is a clinical role focused on improving patient outcomes by identifying and closing gaps in care. You will review patient charts/records, flag overdue preventative screenings and needed specialty consultations, educate patients about their care, facilitate referrals, schedule services, and track/report on outcomes. Your efforts play a key role in preventative health and ensuring patients receive holistic, continuous care.

Duties / Responsibilities

  • Systematically review patient charts in the EMR to detect overdue preventive screenings (e.g. mammograms, colonoscopies, vaccinations, etc.) and any gaps in specialty referrals.

  • Cross‐reference clinical guidelines (e.g. from USPSTF) and organizational standards to ensure preventative care is current.

  • Use clinical judgment to identify patients who should receive referrals to specialties (cardiology, endocrinology, etc.).

  • Perform patient outreach (phone, possibly in person or by other means) to inform, educate, and motivate patients to schedule & attend recommended services.

  • Coordinate and schedule appointments/services, following up to ensure care is completed.

  • Extract, analyze, and report data from the EMR: outreach metrics, gap closing, patient populations, etc.

  • Document all interactions and referrals in EMR; maintain accurate records for continuity of care.

  • Work collaboratively with providers, specialists, administrative staff to facilitate seamless patient care.

  • May help in quality improvement initiatives or population health programs.

Education / Certifications / Licenses

  • Must hold a current, active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in Wyoming (or be eligible for licensure).

  • Relevant certifications (e.g. case management, care coordination, patient navigation) are a plus.

Experience

  • Minimum of 2-3 years clinical experience in a hospital, outpatient clinic, or ambulatory care setting.

  • Experience in care coordination / case management / population health or preventive care strongly preferred.

  • Prior experience using EMR systems regularly, and pulling reports is highly desirable.

Skills / Abilities

  • Proficiency with EMR systems (chart review, reporting, documentation).

  • Strong clinical knowledge of preventative health standards and specialty services.

  • Excellent verbal and written communication skills; ability to explain medical / preventive care concepts in clear, empathetic way.

  • Highly organized, detail oriented, able to manage multiple patients & tasks.

  • Proactive, self‐motivated; able to work independently and collaborate with the care team.

  • Good problem solving; ability to navigate patient barriers (transportation, understanding, scheduling, insurance, etc.).
