This contract opportunity has arisen due to a staffing backfill and offers the chance to work in a supportive, patient-centered environment focused on delivering high-quality care.
Key Responsibilities
- Provide direct patient care in accordance with individualized care plans
- Administer medications and treatments as prescribed by healthcare providers
- Monitor, assess, and document patient conditions and progress
- Assist patients with activities of daily living (ADLs)
- Collaborate effectively with RNs, physicians, and interdisciplinary healthcare teams
- Ensure adherence to facility policies, procedures, and safety standards
Qualifications
- Active Licensed Practical Nurse (LPN) license (state-specific)
- Previous experience in long-term care or skilled nursing facility preferred
- Strong clinical judgment and patient care skills
- Excellent communication and organizational abilities
- Ability to thrive in a fast-paced healthcare environment
