A skilled nursing facility in the San Fernando Valley is looking for a full-time Occupational Therapist to join a well-established therapy department. You'll treat a geriatric caseload across short-term rehabilitation and long-term care, with no weekend requirements and a Monday through Friday schedule. ⠀

This facility is a tree-shaded campus in a residential neighborhood, offering both short-term rehabilitation and long-term skilled nursing care. ⠀
𝗧𝗛𝗘 𝗢𝗣𝗣𝗢𝗥𝗧𝗨𝗡𝗜𝗧𝗬 ⠀
You'll work within a multidisciplinary therapy team that includes Physical Therapists, Speech-Language Pathologists, and therapy assistants, with a Director of Rehabilitation on-site. The facility serves a mix of short-term rehab patients recovering from surgeries, falls, and hospital stays alongside long-term care residents. This is a 13-week contract assignment with strong potential for extension. ⠀
𝗪𝗛𝗔𝗧 𝗬𝗢𝗨'𝗟𝗟 𝗗𝗢 ⠀
- Evaluate and treat residents in activities of daily living: bathing, dressing, grooming, feeding, toileting, and functional transfers
- Develop individualized treatment plans for patients recovering from orthopedic surgeries, strokes, neurological conditions, falls, and general deconditioning
- Perform cognitive and perceptual assessments for residents with dementia, Alzheimer's, and traumatic brain injuries
- Collaborate with nursing staff, physicians, PTs, and SLPs during care plan meetings and daily coordination
- Document evaluations, treatment sessions, and progress notes in Net Health
- Supervise COTAs as applicable and ensure quality of care standards are met
- Utilize available modalities including diathermy, electrical stimulation, and ultrasound as clinically appropriate ⠀
- 𝗤𝗨𝗔𝗟𝗜𝗙𝗜𝗖𝗔𝗧𝗜𝗢𝗡𝗦 ⠀
- 𝗥𝗲𝗾𝘂𝗶𝗿𝗲𝗱:
- Master's degree (or higher) in Occupational Therapy from an accredited program
- Active California Occupational Therapist license in good standing
- Current Basic Life Support (BLS) certification
- Skilled nursing facility experience with adult and geriatric populations ⠀
- 𝗕𝗼𝗻𝘂𝘀 𝗣𝗼𝗶𝗻𝘁𝘀:
- Experience with Net Health documentation system
- Familiarity with the Patient-Driven Payment Model (PDPM) and its impact on OT documentation
- Experience supervising COTAs
- Background in both short-term rehabilitation and long-term care settings ⠀
- 𝗖𝗢𝗠𝗣𝗘𝗡𝗦𝗔𝗧𝗜𝗢𝗡 𝗔𝗡𝗗 𝗕𝗘𝗡𝗘𝗙𝗜𝗧𝗦 ⠀
- 𝗣𝗮𝘆: Competitive hourly rate, commensurate with experience (new graduates and 20-year veterans are not priced the same; your rate reflects your background)
- 𝗦𝗰𝗵𝗲𝗱𝘂𝗹𝗲: Monday through Friday, flexible start times within 7:00 AM to 7:00 PM. OTs can start as early as 7:00 AM to assist with morning ADLs and breakfast routines.
- 𝗡𝗼 𝗪𝗲𝗲𝗸𝗲𝗻𝗱𝘀: Contract therapists are not placed in the weekend rotation.
- 𝗚𝘂𝗮𝗿𝗮𝗻𝘁𝗲𝗲𝗱 𝗛𝗼𝘂𝗿𝘀: 30 hours per week minimum
- 𝗔𝘀𝘀𝗶𝗴𝗻𝗺𝗲𝗻𝘁 𝗟𝗲𝗻𝗴𝘁𝗵: 13 weeks with potential for extension
- 𝗣𝗿𝗼𝗱𝘂𝗰𝘁𝗶𝘃𝗶𝘁𝘆: 85 to 90% for registered therapists. Grouping and concurrent therapy options are available to support this.
