At Home Healthcare, Hospice and Community Services, we are committed to providing compassionate, patient-centered end-of-life care that supports the physical, emotional, and spiritual well-being of our patients and their families. We are seeking a dedicated and empathetic Hospice LPN to join our interdisciplinary team and help deliver high-quality care in homes, hospice facilities, and other care settings.


The Hospice LPN provides skilled nursing care to terminally ill patients, offering comfort-focused care, pain and symptom management, and emotional support. You will work with case managers, physicians, social workers, chaplains, and other members of the hospice team to ensure holistic care for patients and their families.


Key Responsibilities:

· Deliver nursing care based on individualized plans developed in collaboration with the interdisciplinary team

· Monitor and manage symptoms, administer medications, and educate patients and caregivers

· Maintain accurate, timely clinical documentation in accordance with regulatory and company standards

· Provide emotional support to patients and families, facilitating meaningful conversations about goals of care


**This position offers a $10,000 Retention Bonus that will be paid on a quarterly basis throughout the first year of employment**



Requirements:

  • Graduate of a state-licensed basic nursing program;
  • Two or more years of nursing experience, preferably in a home care setting;
  • Possession of a current State of New Hampshire nursing license;
  • Current cardiopulmonary resuscitation (CPR) certification;
  • Valid driver’s license and access to an automobile during working hours that meets the State of New Hampshire registration regulations and auto liability insurance;
  • Sensitivity in human relationships. Able to cope with family emotional stress and tolerant of individual lifestyles. Skill in using psychological principles in both one on one and group situations. Knowledge of measures to comfort the dying. Emotional stability, flexibility, and respect for various religious and spiritual beliefs.
