Job Title: Licensed Practical Nurse (LPN) – Long-Term Care (LTC)
Location: Milbank, SD 57252
Setting: Skilled Nursing Facility / Long-Term Care
Shift: 12-hour Variable Day Shift (6:00 AM – 6:30 PM)
Position Summary
"Navitas Healthcare, LLC" is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide high-quality direct patient care in a long-term care environment. The LPN will be responsible for medication administration, resident assessments, clinical documentation, and collaboration with the interdisciplinary care team to ensure safe, effective, and resident-centered care.
Key Responsibilities
- Administer prescribed medications and treatments in accordance with care plans
- Monitor resident health status and promptly report changes to the RN and/or physician
- Perform routine nursing assessments and document findings accurately
- Provide basic nursing care including wound care and other treatments within scope of practice
- Maintain timely and accurate documentation using the electronic health record system (PCC preferred)
- Assist with admissions, transfers, and discharges of residents
- Support and guide CNAs in delivering resident care as needed
- Ensure adherence to infection control protocols and facility safety standards
- Respond promptly to resident needs, concerns, and emergency situations
- Collaborate effectively with physicians, RNs, CNAs, and other healthcare team members
Required Qualifications
- Active and unrestricted LPN license (state-specific)
- Prior experience in long-term care or skilled nursing facility preferred
- Ability to work 12-hour shifts, including flexibility for scheduling needs
- Strong clinical assessment and documentation skills
- Experience with electronic medical records; PCC system preferred
- Ability to work effectively in a fast-paced, team-oriented environment
