Your Choice Health Care, LLC is currently looking for a RN Start of Care Nurse for the Jackson County area.

- Mon - Fri 8AM - 430PM (on-call might be required)

- Up to $6,240 Sign-on Bonus (Paid over the first year)

What we are looking for:
- Graduate of an approved school of professional nursing.
- license to practice nursing in state of Ohio.
- Current driver's license
- pass a BCI background check and drug screen.
- Good written and verbal communication skills
What you will be doing:
- Provide care and treatment in the patient's home.
- Some of the tasks required: wound care, IV therapy, disease teaching and management, telehealth, medication education and management, venipuncture skills, foley catheter care, assessments and care planning process. Electronic documentation.
- Act as liaison for patient physician and external case managers
- Supervisory visits for paraprofessional staff
