Job Summary
We are seeking a Behavioral Health - Direct Service Specialist to join our team!

As a Behavioral Health - Direct Service Specialist, you will work with our seasoned clinical multidisciplinary team members to assist with coordinating new patient intakes, provide ongoing support services involving the day-to-day care of our behavioral health & SUD consumers and assist with administrative tasks throughout multiple program departments. The ideal candidate will have experience in mental health settings, strong communication skills, work well within a team environment, and be able to engage others in a respectful, professional and empathetic manner.

Responsibilities

  • Assist counselors, nurses & other clinical staff in the day-to-day care of our SPMI client population.
  • Coordinate new client appointments for our Partial Care program ensuring all necessary information is collected prior to intake assessment appointments.
  • Provide support and guidance while building and maintaining relationships with community partners and referral sources.
  • Follow up with new patients prior to first appointment to confirm appointment, transportation and address any questions.
  • Assist Intake Coordinator & Nursing department with obtaining clinical documentation.
  • Coordinate appointments as needed for medical tests & doctors visits.
  • Assist clients with accessing community services.
  • Maintain comprehensive records of patient interactions, promptly reporting concerns as needed.
  • Establish rapport and ongoing therapeutic relationships with clients while building socialization and communication skills.
  • Provide clients with encouragement and positive feedback.
  • Daily utilization of Electronic Health Record (EHR) to document client interactions & maintain ongoing clinical records.
  • Assist with client monitoring during meal service/assist with CACFP meal tracking.

Qualifications/Requirements

  • Strong communication and interpersonal skills.
  • Strong problem-solving abilities.
  • Experience using electronic health records (EHR).
  • Adept in clinical documentation.
  • Ability to work in a fast-paced environment.
  • Minimum BA in mental health or related field.
  • Must possess a positive, empathetic & compassionate attitude.
  • Must have a NJ Driver's license for 5+ years w/ a safe driving record.

Schedule

  • Monday to Friday
  • 7:45 am to 4:15pm
