The MDS Coordinator plays a crucial role in ensuring timely and accurate completion of assessments and care plans in compliance with federal and state guidelines, often managing a heavy workload and prioritizing tasks based on due dates and billing requirements. Their primary responsibility is to accurately assess residents using the federally mandated Resident Assessment Instrument (RAI) process, which helps identify individual resident needs and acuity levels. They coordinate personalized, resident-centered care by effectively communicating assessment findings and care expectations to families and responsible parties, ultimately aiming to maintain timeliness and compliance while delivering high-quality, tailored care.

This position is eligible for a $20,000 sign-on bonus!
Requirements:
40 hours per week - First Shift
8:00 AM - 4:00 PM
Monday - Friday
On-Call Weekend Rotation
Valid RN license in the State of Indiana
BLS/CPR Certification Required
Knowledge of the MDS and the total Care Planning process is required.
Must have good clinical and assessment skills.
Must possess good data entry skills.
