Occupational Therapist – SNF

Overview:
We’re seeking a compassionate and skilled Occupational Therapist (OT) to join a skilled nursing facility team in Riverdale, NY. This role provides the chance to support residents in regaining independence and improving their quality of life through personalized therapy.
Responsibilities:
- Conduct occupational therapy evaluations and deliver treatments in the skilled nursing setting
- Develop individualized care plans tailored to patient goals and needs
- Collaborate with interdisciplinary team members to ensure coordinated care
- Maintain accurate, timely documentation per state and facility standards
- Educate patients and families on therapy goals and home strategies to promote independence
Qualifications:
- Active New York Occupational Therapist license
- Experience in skilled nursing or geriatric care preferred
- Strong clinical, communication, and organizational skills
- Ability to work independently and within a team environment
