JOB SUMMARY
The Licensed Practical Nurse (LPN) plays a vital role in delivering quality care to individuals facing mental health and substance use challenges. Working within HRDI’s Mental Health Division, the LPN is responsible for providing direct patient care, administering medications, conducting assessments, supporting recovery plans, and ensuring compliance with clinical protocols and regulatory standards. This position requires compassion, attention to detail, and a commitment to improving patient outcomes through evidence-based practices and a team-centered approach.
JOB DUTIES
- Conduct comprehensive patient assessments during admission and discharge
- Accurately administer and monitor medications
- Document patient care and follow physician orders
- Track patient progress and note behavioral/clinical changes
- Maintain accurate medication inventory and comply with safety protocols
- Support patients through substance abuse treatment plans
- Collaborate effectively with the multidisciplinary healthcare team
- Other duties as assigned.
QUALIFICATIONS
- Current State of Illinois LPN License with active CEUs
- 2–3 years of experience in substance abuse and/or mental health settings
- Valid CPR certification
- Strong clinical documentation and charting skills
- Proficiency in Microsoft Office and EHR platforms
- Solid knowledge of common drugs, withdrawal symptoms, and related effects
SMART GOALS
- Deliver excellent customer service to all patients and staff daily while employed at HRDI.
- Dispense methadone accurately in accordance with physician orders, SOP, SAMHSA/CSAT, DEA regulations, and accreditation standards daily.
- Complete all required Relias trainings annually by May, ahead of the June 30 deadline.

- Ability to lift up to 50 pounds
- Extended periods of standing, walking, and physical activity

SALARY: $56,000

Occupational Risk for Exposure to TB:
There is an occupational risk for exposure to TB, as this position involves extensive community and patient contact. Employees in this role are required to comply with annual TB testing as part of workplace health and safety protocols. The only acceptable exceptions to this requirement are documented medical contraindications or religious objections. Employees who decline TB testing will be required to wear a mask when deemed necessary.
