Job Summary
We are seeking a dedicated Home Health Physical Therapist (PT) for a field-based role covering the Central Valley / Stockton territory. The PT will provide in-home skilled therapy services to patients, focusing on improving mobility, function, and quality of life.
This role is ideal for therapists who enjoy autonomy, flexible scheduling, and working directly in the community.
Key Responsibilities
- Perform in-home physical therapy evaluations and treatments
- Develop and implement individualized care plans
- Complete accurate documentation using OASIS
- Educate patients and families on exercises and safety
- Coordinate care with nurses and interdisciplinary teams
- Maintain productivity standards (minimum 25 visits/week)
- Travel between patient homes within assigned territory
Required Experience & Skills
- Home Health experience preferred
- OASIS experience required (may consider strong HH without OASIS)
- Strong clinical assessment and documentation skills
- Comfortable working independently in the field
Licensure & Compliance
- California PT license preferred
- Pending CA license accepted (must have applied at submission)
- COVID exemption permitted
Productivity & Pay Structure
- 25 visits = 40 guaranteed hours
- Guaranteed hours only if 25+ visits are completed
- Rate may vary based on territory and experience
Compensation & Benefits
- Mileage reimbursed at IRS standard rate
- Weekly guaranteed hours based on productivity
