Summary: Provide eligibility and enrollment assistance for QUEST/Medicaid, and Federal Marketplace health insurance to all community residents including uninsured patients on Maui, Molokai and Lanai. Serve high risk clients/families with a focus on the COFA community to assure early, ongoing preventive and follow-up care; to provide assistance to those with language barriers and those needing assistance with navigating through the health care and social service systems.
Position Goal: Provide kokua for accessing affordable health insurance services to the uninsured and underinsured Maui County population, naturally including the outer islands of Molokai and Lanai, while targeting COFA, migrant farm workers/service industry workers, homeless, IDUs, LGBTQ community, and recently released prison population. Seeking referrals, cohorts / collaboration with churches and community groups with access to hard to reach & at-risk communities. In doing so we learn that there are families and children who could benefit from other services IMUA Family Services and our partner agencies whom have been overlooked because they have not been a part of the healthcare system in our community.
Essential Duties and Responsibilities:

Complete all training and turn in all paperwork required by DHS, MQD, and FFM.

Possess the ability to perform basic assessments and information gathering techniques.

Possess the ability to gather pertinent data and complete reports as needed and in a timely manner.

Ability to assess and make determinations concerning eligibility criteria for Medicaid, QUEST, HealthCare.Gov (Obama Care) and other private/non-profit/government programs.

Attend meetings as required by HIHAF and MQD, providing timely and informed responses when operational and/or administrative issues or questions arise.

Work well with staff and providers.

Provide prompt assistance to MQD staff regarding operational issues

Assure timely compliance with all contract responsibilities.

Travel to consumers in communities and provide the primary role of assisting people by providing information on affordable health insurance and direct assistance through the enrollment process

Build relationships with other community organizations

Arrange and schedule outreaches in the community for targeted populations through health fairs, individual outreach, etc.

Provide feedback to Program Director on strategies for effective engagement with community members and the type of appropriate tools and measurable amounts for educating specific target populations

Track and record data of numbers of consumers reached through outreach events for COFA, Homeless Population, Farm and Service Workers, LGBTQ Community, Newly Released Prisoners, General Population, Language Assistance Services for persons with Limited English Proficiency. Education activities, consumers reached, consumers served, collateral distributed, consumer support, enrollments for both KOLEA and HealthCare.Gov, 1095A Tax Referral calls/conversations. Including but not Limited to presentations, community events and health fairs, the types and numbers of other consumers’ assistance provided and number of people enrolled.

Submit data monthly to Healthcare Navigator Program Director for tabulation and reporting in a timely manner

Submit mileage and expense reports monthly to the Manager.

Carry out and complete all other duties as assigned by the Manager.

Adjust work schedule when necessary to meet program needs.

Abide by all policies set forth in the employee manual.

Participate in the fundraising activities of IMUA Family Services

May require some supervisory duties of other Community Workers/HC Navigator Staff/Volunteers/Contract Staff • Perform other duties as assigned

Job duties are largely routine and involve little or infrequent independent decision making.
WORKING CONDITIONS (environment)
Indoor air-conditioned office setting predominantly. Occasionally required to be outdoors for travel, and can be exposed to variable weather conditions. Support groups may be off-site in a community meeting space. Occasional inter-island travel by commercial air carrier or boat. Occasional exposure to noise, moisture, noxious odors, and gases. Occasional exposure to dust, dirt, cold, and biological and chemical agents.
Equipment: Frequent use of telephone system and office equipment, i.e. computer, printer, FAX machine, copier, and general office supplies.
Physical Demands: Frequent sitting at desk for office work. Driving of automobile for outside appointments and meetings. Occasional climbing stairs, step stools, occasional stooping, kneeling, crouching, crawling, reaching, and turning. Continuous use of corrected vision, depth perception, wide field vision, and color vision. Continuous use of olfactory, auditory and tactile senses. Occasional pushing/pulling and lifting.
Communication Demands: Frequent talking to public regarding physicians, community agencies, and professionals. Talking on telephone, training/giving instructions, receiving instructions, writing, and composing written language. Ability to work with diverse populations and groups of Maui County, including individual with substance use disorders, mental health diagnosis, HIV positive individuals, and homeless individuals.
Work Hours: Part and Full-time hours available depending on experience.
ABILITIES AND EXPERIENCE:

Ability to speak to and work with a variety of people from various ethnic and socioeconomic groups.

Ability to translate complex information about health insurance to individuals and groups with varied educational levels and backgrounds.

Ability to work with other people in a collaborative manner.

Ability to communicate both verbally and in writing.

Ability to use office equipment, including computers and software packages proficiently.

Responsibility for maintaining client confidentiality and PHI

Effectively trained in cultural competencies

Have transportation, i.e., automobile, etc. which is fully insured and has current registration, safety check, and Hawaii driver’s license.

Ability to work agreed upon scheduled hours –may include some evenings and weekends

Qualifications:

High school graduate or equivalent

Excellent knowledge of the COFA community we serve.

(Advanced/Intermediate/Entry) interpersonal skills required.

Some knowledge of health care language, verbiage and medical terms.

Has ability to work with diverse population.

Has comfort level with the primary care pace and flexibility, including working with interdisciplinary team; strong communication skills; and is a team player.

Has ability to prioritize tasks and work under pressure.

Has ability to work with computers and online utilizing Zoom and other online platforms.

In performing the duties of this job, the employee is regularly required to sit, talk, and hear. The employee may occasionally be required to reach, stoop, bend, kneel, and crouch. Work Environment

Works in a preschool classroom with children around educational material, preschool equipment, toys, and an outdoor playground. Workspace also includes a business office with co-workers utilizing office supplies, computers, printers, copiers, and telephone equipment.
