Our mission is to support a collaborative and diverse workforce where dedication, initiative, and quality care drive excellence.In this role, you will provide individualized nursing care to patients in their homes, ensuring comfort, safety, and high-quality medical support in a familiar environment.Key Responsibilities

• Provide personalized nursing care to patients in their homes
• Assess patient health conditions and monitor vital signs
• Administer medications and treatments as prescribed
• Collaborate with physicians and healthcare team members to develop and implement individualized care plans
• Educate patients and families on medications, chronic conditions, and overall health management
• Perform wound care, rehabilitation support, and other specialized nursing tasks as needed
• Assist with tracheostomy care and suctioning (as required)
• Manage G-tube feedings (as required)
• Administer oxygen therapy (as required)
• Provide daily basic patient care, including repositioning and mobility assistance
• Complete accurate and timely documentation using electronic charting systems
• Offer emotional support to patients and their families
• Perform other duties as assigned

Qualifications & Requirements

• Valid LPN or RN license
• Knowledge of home health safety, sanitation standards, and procedures
• Understanding of confidentiality, privacy laws (HIPAA), and nursing best practices
• Strong understanding of medical terminology
• Ability to learn and use electronic verification/charting software
• Ability to safely turn and reposition patients
• Excellent communication and interpersonal skills
• Compassionate, dependable, and able to handle challenging situations professionally
• 2+ years of nursing experience preferred (not required)
