Job DescriptionRN Care Manager II-IPCM (Non-Exempt) Overview:

The RN Care Manager II-IPCM is responsible for overseeing and coordinating patient care from the moment of admission through to discharge, ensuring smooth and safe transitions after hospitalization. This position entails conducting assessments, developing care plans, advocating for patients, and collaborating with an interdisciplinary team to enhance patient outcomes and manage the duration of hospital stays. Candidates must possess a nursing degree, appropriate licensure, and experience in acute care and care management, with a focus on elevating quality and patient safety in alignment with our organization's mission.

As a Care Manager, you will be an integral part of an interdisciplinary team, responsible for assessing, planning, advocating, and coordinating care throughout the patient’s journey from admission to discharge, ensuring a secure transition following hospitalization. Your responsibilities will be carried out in accordance with our organization's mission, values, and service standards. The Care Management model emphasizes effective transition planning and monitoring of length of stay to enhance patient experience, safety, and quality of care, utilizing performance metrics and best practices.

Position Details:

Required Education:
• Graduate from an accredited School of Nursing is mandatory.
• A minimum of a Bachelor’s Degree in Nursing is required.
Experience:
• Preferred 2-3 years of experience in an acute care hospital environment.
• Experience in Care Management or Utilization Management is preferred.
Licensure:
• A current nursing license in the state of employment is required.
Certifications:
• Basic Life Support (BLS) certification is required at the time of hire or within 90 days of starting.
• Certification in Case Management is preferred.
