About the Role:

The Licensed Practical Nurse (LPN) in Assisted Living (AL) plays a critical role in providing compassionate and skilled nursing care to residents within an assisted living community. This position focuses on promoting the health, safety, and well-being of residents by administering medications, monitoring vital signs, and assisting with daily living activities. The LPN collaborates closely with registered nurses, physicians, and other healthcare professionals to develop and implement individualized care plans. Additionally, the LPN serves as a key point of contact for residents and their families, offering education and support to enhance quality of life. Ultimately, this role ensures that residents receive consistent, high-quality nursing care in a supportive and respectful environment.

Minimum Qualifications:

  • Current and valid Licensed Practical Nurse (LPN) license in the state of employment within the United States.
  • Completion of an accredited practical nursing program.
  • Basic Life Support (BLS) certification.
  • Demonstrated knowledge of medication administration and nursing care procedures.
  • Ability to communicate effectively with residents, families, and healthcare team members.

Preferred Qualifications:

  • Experience working in an assisted living or long-term care setting.
  • Certification in geriatric nursing or related specialty.
  • Familiarity with electronic health record (EHR) systems.
  • Additional certifications such as IV therapy or wound care.
  • Strong organizational and time management skills.

Responsibilities:

  • Administer prescribed medications and treatments accurately and timely according to established protocols.
  • Monitor and document residents’ health status, including vital signs, symptoms, and changes in condition, and report findings to the healthcare team.
  • Assist residents with activities of daily living such as bathing, dressing, mobility, and feeding as needed.
  • Collaborate with interdisciplinary team members to develop, update, and implement individualized care plans.
  • Provide education and emotional support to residents and their families regarding health conditions, medication management, and wellness strategies.
  • Maintain compliance with all regulatory requirements and facility policies related to nursing care and resident safety.
  • Participate in staff meetings, training sessions, and quality improvement initiatives to enhance care delivery.

Skills:

The LPN utilizes clinical nursing skills daily to assess resident health, administer medications, and provide direct care, ensuring safety and comfort. Effective communication skills are essential for interacting with residents, families, and interdisciplinary teams to coordinate care and provide education. Critical thinking and problem-solving abilities enable the LPN to recognize changes in resident conditions and respond appropriately. Organizational skills help manage multiple tasks efficiently, including documentation and medication administration. Familiarity with healthcare technology, such as electronic health records, enhances accuracy and streamlines care coordination.
