Registered Nurse (RN) – Hospice Case Manager
Schedule: Monday–Friday, 8:00 AM – 4:30 PM (No weekends. No on-call!)
Coverage Area: Lancaster & York Counties
Productivity: 5–7 visits per day
About the Role
We’re seeking a compassionate and experienced Hospice RN who thrives in the field and values meaningful patient relationships. This role offers a predictable weekday schedule while providing essential hospice care to patients and families.
What You’ll Do
- Complete admission, recertification, routine, PRN, and death visits in the field
- Perform comprehensive assessments of high-risk patients
- Collaborate with physicians and care teams to create and implement individualized care plans
- Ensure patients receive the most appropriate and cost-effective level of care
- Coordinate care transitions and engage specialty resources as needed
- Arrange non-medical support (transportation, housing, etc.) to help patients follow treatment plans
- Maintain accurate documentation of clinical, functional, and financial outcomes
- Identify opportunities for health promotion and illness prevention
- Act as a patient advocate, protecting privacy and confidentiality at all times
Required Experience
- Hospice experience required
- Experience with Homecare Homebase (HCHB) required
- Ability to manage a productivity expectation of 5–7 visits daily
Licenses & Certifications
- Active, unrestricted Registered Nurse (RN) license
- Valid driver’s license, reliable vehicle, and auto insurance
- Current BLS/CPR certification
Education & Skills
- Associate’s or Bachelor’s degree in Nursing or related field
- Case Management certification preferred
- Strong clinical assessment and critical thinking skills
- Ability to analyze complex medical information and make sound decisions
- Excellent communication and collaboration skills
