Location: South Dennis, MA 02660
Duration: 13 Weeks
Employment Type: Contract
Schedule: Monday – Friday, 8-hour day shifts (occasional weekends & holidays)

Compensation

  • Travel Pay: Up to $2,700/week

Job Summary

We are seeking a dedicated Home Health Registered Nurse (RN) to provide comprehensive patient care in a home-based setting. The RN will conduct detailed patient assessments, develop individualized care plans, and ensure the delivery of high-quality, patient-centered care while collaborating with interdisciplinary teams.

Key Responsibilities

  • Perform comprehensive nursing assessments, including physical, psychological, and environmental factors
  • Develop, implement, and revise individualized patient care plans
  • Evaluate patient conditions and adjust care plans as needed
  • Maintain clinical documentation that reflects skilled nursing care
  • Monitor patient progress and report significant changes to physicians and care teams
  • Administer medications and provide IV therapy as required
  • Participate in interdisciplinary case conferences and care coordination
  • Supervise Home Health Aides and review care plans regularly
  • Educate patients and caregivers on treatment plans and health management
  • Coordinate patient discharges and complete required documentation
  • Ensure compliance with regulatory standards and best practices
  • Refer patients to community resources as appropriate

Required Qualifications

  • Active Registered Nurse (RN) license in Massachusetts (non-compact state)
  • Minimum:
    • 1 year of nursing experience OR
    • 6 months of experience with a BSN
  • CPR certification required
  • Successful completion of pre-employment medication test

Preferred Qualifications

  • Home health or visiting nurse experience
  • IV therapy experience
  • Bachelor of Science in Nursing (BSN)
  • Strong computer and documentation skills
