Duration: 13 Weeks
Employment Type: Contract
Schedule: Monday – Friday, 8-hour day shifts (occasional weekends & holidays)
Compensation
- Travel Pay: Up to $2,700/week
Job Summary
We are seeking a dedicated Home Health Registered Nurse (RN) to provide comprehensive patient care in a home-based setting. The RN will conduct detailed patient assessments, develop individualized care plans, and ensure the delivery of high-quality, patient-centered care while collaborating with interdisciplinary teams.
Key Responsibilities
- Perform comprehensive nursing assessments, including physical, psychological, and environmental factors
- Develop, implement, and revise individualized patient care plans
- Evaluate patient conditions and adjust care plans as needed
- Maintain clinical documentation that reflects skilled nursing care
- Monitor patient progress and report significant changes to physicians and care teams
- Administer medications and provide IV therapy as required
- Participate in interdisciplinary case conferences and care coordination
- Supervise Home Health Aides and review care plans regularly
- Educate patients and caregivers on treatment plans and health management
- Coordinate patient discharges and complete required documentation
- Ensure compliance with regulatory standards and best practices
- Refer patients to community resources as appropriate
Required Qualifications
- Active Registered Nurse (RN) license in Massachusetts (non-compact state)
- Minimum:
- 1 year of nursing experience OR
- 6 months of experience with a BSN
- CPR certification required
- Successful completion of pre-employment medication test
Preferred Qualifications
- Home health or visiting nurse experience
- IV therapy experience
- Bachelor of Science in Nursing (BSN)
- Strong computer and documentation skills
