Job Title: Home Health Case Manager
Pay: $48.69/hr + mileage | Bi-weekly
Job Type:
- Onsite in Belpre, OH
Shift Options:
- Day shift Monday through Friday 8am to 4:30pm with rotating call every 5-6 weeks
Home Health Case Manager Overview:
- Clinical Care & Case Management
- Observe, assess, plan, implement, and evaluate nursing supports for individuals who are well, chronically ill, or acutely ill in community or small-facility settings
- Ensure health maintenance, disease prevention, and timely responses to changes in condition
- Complete daily, monthly, quarterly, and annual nursing documentation as required
- Maintain routine documentation of individuals’ health status in nursing notes
- Ensure progress reports are provided to physicians when an individual’s condition changes
- Transcribe or ensure transcription of physician phone orders and medical reports
Care Coordination & Communication
- Work collaboratively with physicians, healthcare providers, and interdisciplinary team members to plan, implement, and evaluate health services
- Coordinate communication and actions across disciplines related to health and safety supports
- Schedule and track health-related appointments and ensure follow-up actions are completed in a timely manner
- Maintain communication with inpatient nursing staff and discharge coordinators during hospitalizations
Medication Management
- Ensure the ordering, reviewing, monitoring, discontinuation, and disposal of medications as prescribed
- Ensure policies are in place and followed regarding medication administration, errors, storage, access, and destruction
- Review medication-related policies at least annually and revise as needed
Supervision & Leadership
- Supervise and direct licensed practical/vocational nurses as needed
- Provide guidance and oversight to unlicensed staff regarding health-related responsibilities
- Ensure unlicensed staff are trained on person-specific health, safety, and disease prevention needs
- Provide new employee training and annual recertification training on health and safety topics
Health & Safety Compliance
- Ensure furnishings, equipment, and supplies are maintained according to infection prevention and disease control guidelines
- Promote compliance with organizational, state, and regulatory healthcare standards
Home Health Case Manager Qualifications:
Education & Licensure
- Current, unrestricted Registered Nurse (RN) license in the applicable state
- Graduate of an accredited nursing program
Experience
- Experience in community health, case management, long-term care, or small facility settings preferred
- Experience with medication management systems and interdisciplinary care coordination preferred
- Supervisory or leadership experience strongly preferred
Knowledge & Skills
- Strong assessment, documentation, and care coordination skills
- Knowledge of health maintenance, disease prevention, and chronic condition management
- Ability to interpret and implement physician orders
- Effective communication and collaboration skills
- Ability to train, supervise, and support staff
- Proficiency with electronic health records and required documentation
Other Requirements
- Valid driver’s license and reliable transportation
- Ability to travel between service locations as required
- Mileage reimbursement provided at $0.43 per mile
