Full Description of Duties
- As an RN MDS Coordinator/Case Manager, you will complete and assure the accuracy of Minimum Data Set (MDS) assessments for all residents.
- The MDS Coordinator contributes to personalized resident care plans and ensures the capture of clinical reimbursement for services provided.
- The RN MDS Coordinator is responsible for managing short term managed care/commercial cases.
- The RN MDS Coordinator/Case Manager, gathers information, assesses needs, establishes reasonable goals, and provides interventions within an organized, concise, functional care plan.
- Coordinates the completion of comprehensive MDS assessments by the interdisciplinary team and incorporates recommendations in the individual resident care plans.
- Each plan must identify all relevant issues for the care of the resident as well as the goals to be accomplished for each problem or need identified.
- Works together with the care planning team to implement final care plans.
- Encourages the resident and family (or responsible parties) to participate in the development and review of care plans.
- Care plans must focus on assisting residents to reach their highest practicable level of well-being.
- The RN MDS Coordinator/Case Manager ensures that all nursing personnel are aware of the care plan for each resident and that care plans are used in providing daily nursing services.
- Reviews nurses’ notes and monitors the resident to ensure the care plans are being followed and if individual resident goals are being met.
- Review and revise care plans as required.
- Plans, schedules and conducts weekly care plan meetings for all residents according to OBRA and State requirements
Qualifications
- Current Connecticut Registered Nurse license required
- Prior MDS experience required
- RAC Certification is a plus
- Knowledge of Resource Utilization Groups (RUGs), PDPM, Insurance and Case Mix
Compensation & Schedule
- 40 hours a week *On call required*
- Salary commensurate with experience
