Physical Therapist — Outpatient — Centennial, CO

Provide outpatient physical therapy services to evaluate, plan, implement and modify individualized treatment programs. Deliver 60-minute initial evaluations and 30-minute follow-up treatment sessions. Ensure documentation is complete, timely, and compliant with regulatory and clinic requirements. Collaborate with interdisciplinary team members, participate in student education as assigned, and support performance improvement initiatives. Adhere to professional standards, clinic policies, and organizational safety practices.

Minimum Qualifications
\n
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program.
- Active Colorado Physical Therapist license required to start the assignment.
- BLS (Basic Life Support) certification from AHA or ARC may be required.
- Experience in outpatient physical therapy, including conducting comprehensive evaluations and developing functional treatment plans.
- Experience with documentation, goal writing, functional quality indicators, letters of medical necessity, and charge entry in compliance with payer and clinic requirements.
- Ability to supervise and delegate discipline-specific therapy services as appropriate.
- Adherence to state Practice Act and maintenance of professional licensure and required continuing education.

Preferred Qualifications

- Previous travel or contract outpatient physical therapy experience preferred.
- Experience providing student supervision and precepting.

Assignment Details
\n
- Schedule: 5 days per week, 8-hour shifts (5x8).
- Patient visit cadence: 60-minute evaluations and 30-minute follow-ups.
