Job Overview

Work Schedule: Monday to Friday, from 8:00 AM to 4:30 PM, with a rotating on-call schedule during evenings and occasional weekends.

Travel Requirements: Yes, occasional local travel to participants’ residences is necessary.

Compensation: Annual salary ranges from $116,480 to $135,200.

Nurse Practitioner Responsibilities

The Nurse Practitioner (NP), under the guidance of the Medical Director, delivers primary care to participants. Key duties include:

  • Conducting physical assessments for new PACE enrollees and performing semi-annual evaluations.
  • Creating and implementing tailored care plans for participants in the Senior Community Care program.
  • Assessing physical complaints and providing suitable treatments.
  • Educating participants and their caregivers.
  • Collaborating as a member of the Interdisciplinary Team (IDT).
  • Exhibiting the necessary knowledge and skills to care for frail elderly participants in accordance with established standards.

Required Qualifications

  • Must hold a valid Registered Nurse (R.N.) license in the state of employment.
  • Graduation from an accredited nursing program is essential.
  • Certification as a nurse practitioner from a nationally recognized certifying body is required.
  • A minimum of one year of experience working with the elderly and frail population is necessary.
  • Relevant training or experience related to the assigned role is required.

Preferred Qualifications

  • Certification as a Geriatric Nurse Practitioner (G.N.P.) with prescriptive authority is preferred.
  • Two years of experience as a nurse practitioner in a geriatric setting is desirable.

Core Responsibilities

  • Oversee residents in skilled nursing facilities.
  • Evaluate the health status of residents in skilled nursing facilities.
  • Provide medical care and treatment to residents.
  • Coordinate care with Primary Care Physicians and Medical Directors.
  • Conduct comprehensive history and physical examinations for new participants.
  • Work with the Interdisciplinary Team to develop comprehensive care plans.
  • Perform semiannual reassessments and additional evaluations as needed.
  • Integrate primary care treatment plans into the overall care strategy developed by the IDT.
  • Engage with team members to address urgent needs of participants and assist in discharge planning.
