Our organization operates Skilled Nursing Facilities (SNFs) that offer:
- Long-term care for residents in need of ongoing support.
- Short-term, sub-acute rehabilitation for individuals recovering from surgery, illness, or injury.
- Collaborative care through a multidisciplinary team approach involving nursing, therapy, and medical professionals.
The patient demographic may vary slightly across different facilities. As a therapy professional, your role will be to assist patients in achieving functional improvements and enhancing their quality of life within a nurturing, patient-focused environment.
Key Responsibilities:
- Deliver direct rehabilitation services in speech therapy to patients based on physician referrals, adhering to all relevant laws, regulations, and organizational standards.
- Engage in community health initiatives and projects as appropriate.
- Utilize therapy software effectively and accurately.
- Provide oversight and evaluation of care delivered by Clinical Fellowship Year (CFY) SLPs, if certified.
- Implement and interpret departmental programs, goals, objectives, policies, and procedures.
- Maintain confidentiality regarding all proprietary and sensitive information.
- Exhibit excellent customer service and maintain a positive demeanor towards patients.
- Read, write, speak, and comprehend the English language proficiently.
Qualifications:
- Graduation from an accredited speech therapy program (minimum MS or MA in SLP).
- ASHA certification (CCC) or eligibility for a 9-month Clinical Fellowship Year (CFY) to obtain ASHA certification (CCC) and/or meet ASHA credentialing criteria. Ongoing maintenance of ASHA credentials is required.
- Proficiency in Microsoft Word, Excel, and email communication.
- Familiarity with practices, procedures, and regulations governing operations in post-acute care facilities.
- Demonstrated ability to perform competently in all relevant areas, including patient rights, safety, and sanitation.
- Understanding and adherence to organizational policies, including those related to harassment and compliance.
- Display integrity and professionalism by following the organization's Code of Conduct and completing all mandatory compliance training.
