Your Role as a Home Health Physical Therapist:
- Provide specialized physical therapy according to a personalized Treatment Plan created in collaboration with the referring physician.
- Assess patient requirements and set rehabilitation objectives aimed at enhancing mobility, strength, balance, and overall physical capabilities.
- Modify treatment strategies and approaches as necessary to ensure the best possible results.
- Work alongside a multidisciplinary home health team—including nurses, occupational therapists, speech therapists, and social workers—to deliver integrated care.
- Instruct patients and caregivers on exercises, safety measures, mobility techniques, and the use of assistive devices.
- Accurately document all visits, assessments, and progress notes in a timely manner.
- Assist patients in maximizing their independence and enhancing their quality of life—no matter where they reside.
About You: Qualifications – What You’ll Bring:
- A Master’s degree in Physical Therapy from an accredited institution.
- A current PT license in the state of employment (or eligibility to obtain one).
- CPR certification is mandatory.
- Excellent communication, assessment, and problem-solving abilities.
- Capability to work autonomously in diverse home settings.
- A valid driver’s license, auto insurance, and dependable transportation.
Preferred Experience (Not Mandatory):
- Over one year of experience in a hospital, home health, or outpatient rehabilitation environment.
- Experience delivering therapy in home settings.
- Background in geriatric rehabilitation, orthopedic, or neurological rehabilitation.
- Experience in post-acute care or chronic condition management.
- Collaboration within a mobile or interdisciplinary care team.
