Our organization is on the lookout for a committed Physical Therapist to become a part of our Healthcare & Medical Services team. This role focuses on delivering compassionate, evidence-based rehabilitation services primarily to senior residents within our facility.
Key Responsibilities
- Conduct thorough evaluations of patients to assess their physical therapy needs.
- Develop personalized treatment plans aimed at restoring function, alleviating pain, and enhancing independence and quality of life.
- Implement skilled interventions, including therapeutic exercises, manual therapy, gait and balance training, and appropriate modalities.
- Collaborate closely with an interdisciplinary team, which includes nursing, occupational therapy, speech therapy, social services, and physicians, to ensure comprehensive, patient-centered care in a family-oriented setting.
- Document patient progress accurately in the electronic medical record system.
- Communicate effectively with patients and their families regarding treatment plans and home exercise routines.
- Participate in care plan meetings and discharge planning to facilitate safe transitions of care.
Qualifications and Requirements
- Valid physical therapy license.
- Strong interpersonal and communication skills.
- Ability to work collaboratively in a team environment.
- Commitment to following all regulatory, safety, and infection control standards.
