About PACE
Philanthropists Gary and Mary West founded West Health with the mission of enabling seniors to successfully age in place with access to high-quality, affordable health and support services that preserve and protect their dignity, quality of life, and independence. The Gary and Mary West PACE (Program of All-inclusive Care for the Elderly), affiliated with West Health (www.westhealth.org), is a non-profit that delivers medical, social, and behavioral services to the frail elderly to the North County of San Diego. This PACE center is entrepreneurial in nature and focuses on innovations and bringing new processes to the traditional PACE environment.

Position Summary:
The Gary and Mary West PACE (GMWP) Assessment RN is responsible for conducting initial nursing and home safety assessments of prospective participants at their place of residence and managing State level-of-care (LOC) review process with Department of Health Care Services nurse. The Assessment RN is a member of the Gary and Mary West PACE Clinic and Marketing Team and works in close collaboration with the PACE IDT team, marketing specialist, and prospects' caregivers to thoroughly and promptly evaluate each prospective participant interested in PACE enrollment.
Essential Functions
•Conduct initial nursing assessments, including but not limited to nursing evaluation, functional assessment, mini mental status, home safety evaluation, in prospective Gary and Mary West PACE participants' homes, to determine if they meet PACE eligibility requirements.
•Promptly prepare documentation from home visits, submit to CA State Level-of-Care (LOC) nurse for review and respond to questions to expedite review process for participants.
•Track and manage the annual State level-of-care review process.
• Comply with the Marketing Team's compliance with all rules pertaining to allowable and prohibited marketing practices.
•Cross-train on other Marketing Team roles and provide back coverage as directed.
•Provide clinical coverage and backup to the nursing team, at clinic, home or alternative care sites including RCFE, ADHC and Congregate living communities.
•Deliver and document nursing interventions as agreed upon in the participants' care plans including but not limited to maintaining a healthy and safe environment, promptly and accurately responding to physician orders, and correctly administering medications and performing ordered tests and treatments.
•Collaborate with the Gary and Mary West PACE IDT and Marketing Team members.
•Complete all required documentation in a timely and accurate manner, including in CRM software, Salesforce.
•Protect privacy and maintain confidentiality of all company procedures and information. about employees, participants and families.
•Follow Gary and Mary West PACE policies and procedures and participate in any required Quality. Improvement activities, staff training and meetings.
•Other duties as assigned.
Education and Certification:
•Graduate of an accredited school of nursing; Bachelor of Science (BSN) preferred
•Unencumbered California Registered Nurse (RN) License
•Current Basic Life Support (BLS)/ First Aide Certification required
•Original valid driver's license with proof of auto insurance.
Skills and Experience:
•Minimum of two years nursing experience in a clinical setting with a frail or elderly population.
•Nursing knowledge and skills necessary to treat frail, elderly participants and manage complex clinical situations.
•Highly motivated, self-directed, able to execute tasks in a quickly changing environment and make sound decisions in emergency situations.
•Excellent clinical, organizational and communication skills in settings with seniors, their families and interdisciplinary team members.
•Experience and competency working with people from diverse backgrounds.
•Commitment to unlocking the full potential of our most vulnerable seniors.
•Bilingual Spanish-English preferred.
Medical Clearance:
Employees must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
