Occupational Therapist (OT) – Home Health We are seeking a dedicated and compassionate Occupational Therapist to join our team in the Houston area. The ideal candidate will provide high-quality care to patients experiencing physical, cognitive, developmental, or emotional challenges that impact their ability to perform daily activities. This position offers flexibility with full-time, part-time, permanent, and temporary options available. Key Responsibilities: - Evaluate patients' functional abilities, limitations, and needs through comprehensive assessments.- Develop individualized treatment plans designed to improve patients' independence and quality of life.- Implement therapeutic interventions focused on activities of daily living (ADLs), instrumental activities of daily living (IADLs), cognitive skills, upper extremity function, and adaptive techniques.- Monitor patient progress and modify treatment plans as needed to achieve optimal outcomes.- Educate patients, families, and caregivers on home exercise programs, adaptive equipment, safety strategies, and techniques to maximize independence.- Maintain accurate and timely documentation of evaluations, treatments, progress notes, and discharge summaries in compliance with regulatory requirements.- Collaborate effectively with physicians, physical therapists, speech-language pathologists, nurses, and other healthcare professionals to coordinate patient care.- Participate in continuing education and professional development activities to remain current with best practices in occupational therapy. Qualifications: - Bachelor's, Master's, or Doctorate degree in Occupational Therapy from an accredited program.- Current state licensure or eligibility for licensure as an Occupational Therapist in Texas (must be obtained prior to employment).- Previous home health experience preferred but not required; new graduates are welcome to apply.- Strong verbal and written communication skills with the ability to establish rapport with patients and families. Skills: - Knowledge of rehabilitation principles related to activities of daily living, cognitive function, and adaptive equipment.- Experience using electronic health records (EHR) systems.- Ability to work independently while collaborating effectively within a multidisciplinary team.- Compassionate, patient-centered approach to care with a commitment to improving patient outcomes.
