Job Summary

We are seeking an experienced and compassionate APRN/Nurse Practitioner to join our Adult Medicine clinic. The ideal candidate will provide comprehensive primary health care services, including health promotion, disease prevention, diagnosis, and management of acute and chronic conditions. This role involves collaboration with physicians and other healthcare professionals to ensure high-quality, patient-centered care.


Key Responsibilities

  • Deliver comprehensive primary care services to adult patients.
  • Promote health and prevent disease through patient education and counseling.
  • Collaborate with physicians and refer patients to specialists when necessary.
  • Obtain and document detailed medical histories and perform physical examinations.
  • Diagnose and manage acute and chronic health conditions.
  • Prescribe medications and manage treatment plans in accordance with state and federal regulations.
  • Maintain accurate and timely clinical documentation in the electronic health record (EHR).
  • Provide culturally competent care to a diverse patient population.
  • Participate in quality improvement initiatives and comply with organizational quality measures.
  • Perform other duties as assigned by the department or organization.

Qualifications

  • Graduate of an accredited School of Nursing and an approved Nurse Practitioner program.
  • Current and valid APRN licensure in the State of Connecticut.
  • Eligible for or in possession of DEA and state narcotic registration.
  • 1–2 years of experience in an ambulatory or urgent care setting preferred.
  • Strong knowledge of nursing theory and clinical practice.
  • Proficient in taking medical histories, assessing conditions, and interpreting findings.
  • Excellent communication skills, both verbal and written.
  • Demonstrated ability to make evidence-based clinical decisions.
  • Comfortable using EHR systems and other healthcare technologies.
  • Commitment to providing high-quality, patient-centered care.
